I took in an elderly patient with advanced Alzheimer's disease because he appeared to have had a seizure. He was not examined on the basis of this possibility, but received an oral interview instead, at which I acted as facilitator because of the patient's deteriorated cognitive function. Staff worked hard to appear obliging, but it all seemed oriented to the fulfillment of a job description as opposed to genuine probing. No examination had been done prior to the interview to determine whether a seizure had taken place, and no blood was taken for analysis while I was present. Instead, after a long wait we were apprised of various at-home programs of which we might avail ourselves. All information was delivered in a rapid manner such that it was difficult to apprehend. No concrete resolution was arrived at and the patient continues to deteriorate. Today we are consulting elsewhere and examining other options. We shan't return to Southlake - it has become too big, a labyrinthine quagmire too difficult to navigate. In the end, my elderly friend was traumatized by the events of the day, and I spent the night trying to keep him calm so he could get to sleep. One positive note: the "goodwill ambassadors" at the ED appeared to have a genuinely positive effect. Their function is not immediately apparent, but they worked to reassure patients and were able to facilitate a few requests. Final note: at least two cases of COVID-19 were confirmed while we waited (3 1/2 hours) - the people in the room in which we were waiting were visibly alarmed at hearing staff discuss the cases in loud voices out in the open. It would be rather more professional for staff to discuss such matters more discreetly, out of courtesy for the rest of us sitting and waiting to be seen.
